Cancellation Policy

Neuropsychological Evaluation Booking Policy Dr. Zackery A. Tedder provides comprehensive neuropsychological evaluations for individuals seeking diagnostic clarity related to cognitive, attentional, emotional, and psychological functioning. These evaluations are designed to assist with diagnosis, treatment planning, accommodations, or legal documentation when appropriate. Scheduling Requirements All evaluations require a scheduled intake session prior to test administration. Full evaluation appointments (testing + feedback) are typically booked 2–4 weeks in advance, depending on availability. Requests for expedited evaluations may be accommodated on a case-by-case basis and may include an additional priority fee. Fee Structure • Evaluations are billed at a flat rate determined by referral question and scope of testing. • A non-refundable deposit is required to secure your appointment. • Final payment is due prior to the release of any written report or feedback session. Dr. Tedder does not bill insurance directly but will provide a superbill upon request. Cancellation & Rescheduling Policy Testing appointments require at least 72 hours’ notice to cancel or reschedule without penalty. • Cancellations made within 72 hours will result in a $250 late cancellation fee. • No-shows or day-of cancellations will result in forfeiture of the deposit. If you arrive more than 20 minutes late to your testing appointment, the session may be rescheduled and subject to a rescheduling fee. Documentation and Turnaround Time Final reports are typically delivered within 2–3 weeks following completion of all testing components and any required collateral. If documentation is needed for school, work, or legal purposes, please indicate this at intake so timelines can be discussed in advance.
